Persistent or recurrent urinary incontinence after the placement of the artificial urinary sphincter (AUS) requires a systematic investigation to elucidate the exact etiology of failure. Placing the second cuff around the bulbous or membranous urethra doubles the surface area that is compressed, offering greater resistance to urine leakage without decreasing tissue perfusion. This technique represents an alternative to use of a smaller cuff or a higher pressure balloon.
We used this approach in 25-year-old patient with neurogenic bladder and persistent urinary incontinence after the AUS implantation.
