The use of two-phase CT-angiography in the surgical treatment of renal tumours

Objective: The aim of the work is to evaluate the first experience with the use of two -phase CT-angiography (CTA) of kidney at the surgical treatment of parenchymatic renal tumours.

Material and methods: In 15 patients two-phase CTA kidney vessels was consequently indicated from October 2003 to July 2004, by means of computer tomography at Somaton Sensation 16 equipment, produced by Siemens. The findings at CTA were compared to the pre-operational finding (9x at laparoscopic nephrectomy, 3x at transperitoneal nephrectomy, 2x at translumbal tumour resection, 1x at retroperitoneoscopic nephrectomy).

Results: In 4 patients there were 2 renal arteries visible (27%) - 2x on the right side, 2x on the left side. Two renal veins were found two times on the right side (13%), once there was an early division of the left renal vein. Having compared the anatomic finding at the arteries as well as at the veins, in all the cases the radiological picture was identical to the preoperational finding. The picture of aberant veins going from the area of right ovarial vein to the tumour in the area of right kidney lower pole in 4 patients (27%) was also surprising.

Conclusion: Multidetector spiral CT enables, among other things, to make two-phase CTA, an excellent method of preoperational manifestation of vessel supply of the kidney. It is able to replace classic angiography, in addition there is also displayed the vessel supply and we get precious anatomically- topographic data. Multidetector spiral CT including two-phase CTA should be made at renal tumours routinally immediately after the sonographic examination. If it is available in given location, of course.
