We present two case reports of patients with Leydig cell tumors of the testis. These were the only two cases diagnosed at our department since 1989. The first patient had a benign form, and the second patient, an elderly man, had a tumor of uncertain biological character. We describe the diagnosis, treatment and surveillance strategies. In addition, we discuss the possibilities of organ-sparing surgery. We point out the need for monitoring all patients for possible malignant transformation of originally benign forms of this tumour.
