Transurethral injection of botulinum toxin in the treatment of neurogenic hyperactivity of the detrusor muscle

Neurogennic hyperactivity of the detrusor muscle is one of the typical consequences of the impairment of innervation of lower urinary tract at the level of upper motor neuron. The depression of the detrusor muscle activity with anticholinergics together with intermitent catheterisation is used in treatment, surgery is used in refractory cases.
Botulinum toxin A is a neurotoxic substance isolated from Clostridium botulinum, causing the blockade of release of acetylcholin from presynaptic membrane.
The aim of the study is to prove the effectivity of botulinum toxin treatment in patients with neurogenic hyperactivity of the detrusor, resistant to conventional pharmacotherapy.
9 patients with neurogenic hyperactivity of the detrusor muscle were included. 800 -1.000 UI of botulinum toxin A was injected to 40 places of the detrusor muscle.
7 patients were satisfied with the effect of the therapy, 2 reported a marked improvement of their condition. Continence was restored in 7 patients. The control urodynamic investigation revealed na increase of volume before the first udepressed detrusor contraction (124...482ml), the incease of maximum cytometric capacity
