Physical and ultrasound examination revealed a tumor mass in left hemiscrotum. There was high suspicion for malignant tumor of left testicle, therefore, a left radical inguinal orchidectomy was performed. Laparoscopic adrenalectomy was performed, and histopathological examination confirmed a cavernous lymphangioma. It is a benign tumour that occurs in most cases in the head and neck region or in the mediastinum. The primary localisation in the adrenal gland is rare.

Urethral duplication represents a spectrum of rare congenital anomalies with different phenotypes, the common feature of which is the duplication of a part or complete urethra.
