Invasive aspergillosis is serious complication of infection caused by aspergillus which occurs in immunocompromised patients. Authors present case report of aspergilloma of epididymis in young patient with acute myeloid leukemia. After unsuccessful conservative treatment surgical solution was chosen.

The aim of the study was detailed description of the diagnostic method and analysis of Doppler waveforms of arteries supplying scrotal content in physiological conditions. Sixty seven men and adolescents without signs of scrotal content disorder were investigated in 1997-2003. The population was divided into 3 age groups: under 20 years, between 20 and 50 years and over 50 years. Aim: Skin and mucosal grafts or skin flaps are used for surgical reconstructions in patients with anterior urethral strictures. Their use is accompanied with increased morbidity from donor site or from flap development. SIS is acellular heterologous matrix available as a potential tissue substitution for urethral reconstructions. Didymal tumours from leydig and sertoli cells

The authors present their own experience with diagnostics and treatment in five patients with Leydig cell tumors (LCT) and one patient with Sertoli cell tumors (SCT). Ultrasonography appears an excellent diagnostic method in testicular tumors. Blood markers (AFP, ß-hCG), estradiol and testosterone help to confirm diagnosis of LCT and SCT. Radical orchiectomy is the initial treatment of choice. Retroperitoneal lymph node dissection is reccomended for malignant lesions with nodal involvement because of chemoresistance and radioresistance. All patients must be observed for a long period.
Case history duration and symptomatology in prognosis of renal carcinoma

High erythrocytes sedimentation rate (p = 0,04), anemia (p = 0,01) and especially systemic toxic tumor symptoms (p < 0,01) were markers of significantly worse prognosis. Prognosis of patients with pain (p = 0,80) and fever (p = 0,83) was not different from survival of patients with hematuria. Prognosis with palpable mass (p = 0,13), classic tumor trias (p = 0,09) and hepatopathy (p = 0,38) was only insignificantly worse. On the contrary, patients with erytrocytosis (p = 0,82) and with hypertension in connection with tumor (p = 0,12) had only insignificant better survival. Between 1999 and 2003, 61 patients (40 men and 21 women - average age 58,5 years) were treated for advanced renal cell carcinoma in the Department of Urology, 2nd Faculty of Medicine, Faculty Hospital Motol. Adjuvant therapy was administered in 22 patients (13 men and 9 women -average age 55,1 years) after radical nephrectomy for renal cell carcinoma without documented metastatic disease but with high risk of relapse. In the group of 316 patients operated for renal cell carcinoma were 191 men and 125 women (1,5 : 1). The five-year survival of men and women was 59,7% and 67,2%, respectively. The difference was not significant (p = 0,18). Distant metastasis had 14,1% men and 12% women. There were 6,3% inoperable tumors in men and only 1,6% in women. Frequency of incidental carcinomas was insignificantly higher (p = 0,24) in women (31,2 vs 25,1%). In the retrospective study, five-year survival in the whole group of patients with renal cell carcinoma (RCC; n = 316) was 62,7%. In the group of patients after tumor removal (n = 302) and in the group with RCCs T1-T3b N0 M0 (n = 250) was 65,2% and 76,4%, respectively. Considering the size of the tumor, five-year survival after removal of RCC (n = 302) in patients with tumors < 5 cm, with tumors .5 cm and < 10 cm, and with tumors .10 cm was 83,1%, 59,4% and 54,5%, respectively. Primary paraganglioma of urinary bladder - case report and the disorder

Presented is 57-year-old woman who suffered by hypertension and dysuria with macroscopic hematuria. Endoscopic appearance of a large intramural tumor simulated high-stage urothelial malignancy. The case was sent for consultation in our lab and diagnosis of primary pararaganglioma of the urinary bladder was eventually rendered. Performed was wide excision and finally partial cystectomy. The patient is now 3 month free of disease. The literature is reviewed and morphological differential diagnosis is discussed in sight of biologic, prognostic and therapeutic implications.
Benign retroperitoneal schwannoma - case report

Authors present a case report of 53 years old man with benign retroperitoneal schwannoma localised below the lower pole of the left kidney. Primary percutaneous biopsy has failed to make correct diagnosis. The patient underwent retroperitoneal surgery and removal of the tumor. Neurological complication - sensory and short-time motoric dysfunction in the femoral nerve area followed the surgery. This complication completely disappeared after physiotherapy after short period. The patient is free of any problem and satisfied now.
Radical laparoscopic nephrectomy in tumour with tumorous vascular thrombus (T3B)

Introduction: Renal tumours T3b, i.e. with a thrombus in renal artery or in vena cava inferior (VCI), are considered to be a contraindication for laparoscopy. The aim of our study was to provide new knowledge about this problems.Our population: We made 71 radical laparoscopic nephrectomies or retroperitoneoscopic nephrectomies due to parenchymal tumour between January 2003 and June 2005. Class pT3b, i.e. tumour spreading to renal veins proved by histopathology, was found in 16 (22,5 %) patients.
