The indication of radical nephrectomy in renal cell carcinoma clinical stages III and IV

A retrospective study compared cohorts of patients with stage III and IV renal adenocarcinoma treated at the Clinic of Urology between 1988 and 1999. 122 patients who underwent surgery and 65 patients treated conservatively were evaluated.
By comparing the adjusted survival of stage III patients with or without radical nephrectomy, a survival 49 months longer was found in those who had undergone surgery; however, the difference was not statistically significant (p = 0.200). The same comparison was made for stage IV patients which showed a survival of 18.1 months vs. 14.6 months in favour of those undergoing radical nephrectomy. No statistical significance was found either (p = 0.215).
Based on the results obtained, surgery can be recommended in an advanced renal carcinoma of stage III. Stage IV metastasis-free patients can also benefit from surgery. It is indicated in pulmonary metastases with possible subsequent treatment. In the case of distant metastases in multiple organs, surgery is mostly of no benefit to the patient.
