Abdominoscrotal hydrocele is a rare disease of unknown etiology. It consists of the scrotal and abdominal sacs that communicate though a patent hole in the groin, shaped like an hourglass. The occurrence of this condition is very rare in children, however in adult men this finding is extremely unusual. Diagnosis is based on clinical examination and ultrasound or CT. A confirmed diagnosis necessitates surgical treatment. In this case report we present a case of voluminous abdominoscrotal hydrocele in an adult 21 year old male. In this case, the problem was discovered after reaching sexual maturity. We describe the history, physical examination, clinical imaging methods used to confirm the diagnosis and select surgical intervention.
